Unnecessary "Require: 100rel" header in re-INVITE request #303

It is observed that when when A initiates an audio call with B and B converts 
it into video call, it (Party B) sends Require: 100rel header with re-INVITE, 
which has no significance. It is used in a case where provisional responses 
(18x) were to be acknowledged. In case of re-INVITE, there can't be any 
provisional response like (18x).
