README: add Table of Contents #55

The original README is big enough. Every time you want to find a particular place in it, you need to scroll the whole text. Table of Contents to the rescue.

Hi - apologies for not getting back to you on this, I have been travelling without much IP access over the last 3 weeks. A TOC would be useful to add. Would you have a strong objection to placing it after the Description section? This ensures that even on small devices potential users get to see a screenshot in their first few pages.
