Update documentation to reflect current Ionic native usage #36

The documentation in this repo does not reflect the proper Ionic native usage. I updated that, and cleaned up some of the other formatting in the readme.

I submitted a change to the Ionic native wrapper project as well to reflect these same changes: https://github.com/ionic-team/ionic-native/pull/2587.
