Case view style changes #17

  1. added account/resolution fields to case view
  2. changes display blocks to have
    • a top section for smal field, 2 per lines
    • a main section with full width fields for subject, description and resolution.
  3. changed button style.
  4. Added case list view css entries to break the table in blocks for small screens.
