Include subcategories always checked in items manager tab #315

When clicking to unselect this checkbox the reloaded page has the checkbox 
checked again

The function getUserStateFromRequest() will not function properly with the 
checkbox form field, because when checkbox is not checked it POSTs no value, 
thus the previous value is maintained.

Solution is to replace with a radio button, that has 2 values ...
