1) There are a number of character descriptions that begin with =. While I don't have time to write a description for each character, I could search/replace to get rid of the = signs at the start. Example:
\acute =acute accent above x
\breve =breve above x
\check =caron above x
2) Multiple characters (character with combining accent) in the MathML spec are not correctly converted back to text. Example is 'not right-arrow curved' U+2933 U+338.
3) In general there are a number of characters that are defined multiple times in the spec. The last one takes effect, but this is sort of random. It should at least be indicated.
