Implement the ToConcept Function and Equivalence on Concept #1872

In order to https://github.com/medizininformatik-initiative/sq2cql/issues/117 to work with Blaze, we need the ToConcept function that converts a FHIR CodeableConcept into a CQL Concept and the equivalence operator on concepts.
