Build outputs for non mp4 outputs #37

Output file detection simplified by creating outputs subdirectory. Label detection also changed. No new tests written but existing tests are passing.

This was tested in a demo hyrax app through hyrax-active_encode and it works! A valid manifest with two outputs (the same as stock hyrax) are created and the avalon player displays and plays! :tada:
