Cleans up the A-Z index #336

Fixes #334

Removes a-z entries that duplicate page titles
Adds more descriptive terminology to existing terms

Note: the a-z index is generated based on the a-z field in the front matter and the title of the page. There are instances where pages have the same name, and I was unsure what to do about those, but this is a step forward.
