Add support for mssql-to-postgresql migration #13

For this change, the project name should be changed as well and also we should consider other platform(s) migration which could be added in future. Initially I'm thinking names like

db2db
DbToDb
DbMigrator
NetDbMigrator
DataWizard
TheMigrator
MigWiz
