Detect need for short-forms #2

Right now, short forms are only used in subsequent citations. I need to have the first mention render the short-form if it will be used subsequently.

Short forms are now rendered within first citation and only when subsequently used (05cd5b2a9ce4fd9f912878587a20ac5a3475a236). But, they're shown when not needed (e.g. when the short form is the default short form, like the author's last name).

Now, when a short form matches the default short form, it is not specified alongside the first citation (b9ad084c61d01865f49bb568b3b24858d3630f13).
