Add back support for restricted user access to selected layers of the deployment. #171

There should be a config.php setting that makes the code only allow
registered users to have access to the site. This can then be configured on
a layer-by-layer basis.

The default should be to allow public access and open registration on the
production layer, and to have closed access on the QA and development layers.

The code will default to open access, so if the setting is missing, it will
allow access. The parameter must be set in order to restrict access.

This is an essential feature when reviewing new media for objects that may
be culturally sensitive, or otherwise problematic to expose without review
beyond a qualified audience.
