Build in feedback system #25

There should be a way for people using the site to easily submit feedback. 

Initial ideas:

- Keep the "button" visible on every page
- Over lay the feedback form on top of the page on which the feedback button 
was pressed
- Allow uploading of screenshots
- Record the url on which the feedback was submitted
- Record the current logged in user, if there is one

Need a simple means for users to provide feedback on a details page. Should be 
something like a form-mail popup with select for subject (allow "other"), and 
simple 
body text-entry. Submit sends mail to us. Consider what subject categories to 
have, 
ensure that submit embeds the object number somehow.

There is a very simple contact form linked in the footer. Something more 
elaborate should be done if time 
allows.

One small step would be to recognize a logged in person and pre-fill the form 
for
them, or even provide an alternate form that makes these non-editable.

Updated to pre-fill name and email for logged in users.
Removing Milestone1 label from this as we have much of what we need.
Would still be nice to have direct feedback integration into other pages as 
well, but
that is not critical for this release.
