Not sending all information

Following all the steps for 3.1.1.

Only the timestamp and package name are sent.  The rest of the default data 
does not appear in the spread sheet.

Duplicate of Issue 33.  The form was create improperly.  Perhaps this should be 
added to the docs:

1) After importing the Template open it. 
2) In the open file go to Form->Create
3) Rename it or change something to that it can be saved
4) get the key from this form and use it in the application

I'm ok to fix the doc if it is not clear I but don't understand the difference 
between what is in the current doc 
(http://code.google.com/p/acra/wiki/ACRA3HowTo) and your proposal.

Could you please explain what was your error exactly ?

So heres where the confusion seems to be.

   - Open the imported document
   - Rename it as you like
   - In the menu, click on Forms / Create form

To me this implied Open the imported document and rename it (if desired).
Then, "In the menu, click on Forms / Create form", seemed to mean in the
Google Doc's menu (Create New -> Form ) which is not what we need to do.
 Just needs some clarification.
