Cosmetic: rename from "internal" -> "associated" headers #115

Parts of IWYU (including tests) refer to associated headers as 'internal' 
headers. This patch changes everything to consistently use 'associated headers' 
for foo.h and foo-inl.h in relation to foo.cc.
