Fix #136.

This PR includes the following updates.

Migrate @grafana/toolkit to @grafana/plugin-tools.
The front-end component of the function parameter is updated with the Segment UI component.
The PV Name and Operator front-end components are updated with Select UI components.
The GitHub workflows are updated according to the workflows in @grafana/plugin-tools.
