I applied some style from the SBS chanting book that I thought might help.
removed the -handbook.tex files
In the Bhikkhu Manual these were used to deal with content variation between editions.
added the chapter opening images you have in the SBS file
I extracted the pages from the SBS PDF with pdftk. In the future I suggest creating these decorative pages in Inkscape, and saving the PDF to include with \includegraphics.

Bold and Small Caps, as in the SBS book, with letter spacing (see \SetTracking, docs in the microtype package)
