Improve documentation and readme #38

I've taken a stab at improving the documentation for the default hooks.json as well as improve the readme. This would have helped me a lot when I started out.

I haven't strictly speaking tested the new hooks.json (aside from validating the schema) so please let me know if it contains any errors.
