Switch to BibTeX file for all references #11

All documents should use a BibTex file, for simplicity and consistency.

Added bib.bib file and used in doc 100 in 9a0766f13af9b7d97e91f64304e1ed51b39c548a

I am trying to finalize an approach for citations and references so we can apply it to all documents and move on. @soweinrich (as a LaTeX user) could you take a look at document 100 (including citations and hyperref stuff) and bib.bib if needed in the revising branch (commit 785f750b37b44f89415311b4211486edc23143d3 +) and let me know what you think of the current approach? I have tried several options, and think this combination is a good choice (and did not get dinat style to work reliably). Feel free to try other options.

Note that the citation keys are auto-generated by Zotero using the Better BibTeX add-on, except for the BMP documents, which I manually entered (otherwise all 200 docs look similar).

This BibTeX stuff seems to be working OK for now. Closing issue.
