Menu items appear in irrelevant popup menus #77

The menu items of the GFM Viewer plugin are contributed to irrelevant popup menus. This happens when there is no selection in the Project Explorer view or a Markdown file is selected.

I believe the reason is that the menu contribution definition in the plugin.xml is not completely accurate.
