Port to Dev15 and reorganize repo

This pull request ports SlowCheetah to Visual Studio 2017 and reorganizes the repository according to the following project structure:

SlowCheetah: Main transformation logic and build task
SlowCheetah.VisualStudio: Visual Studio extension for applying and previewing transformations
SlowCheetah.UnitTests: Unit tests for the

The changes simplify and clean up the repository

Also, the previous package installation (directly into AppData folder) was removed and replaced by a pure Nuget package for the main SlowCheetah.

I'm confused, just checked Extensions & Updates and it wasn't there but it says Closes #201

@cilerler the new version will be published to NuGet and VS Gallery early next week.
