modifying link causes second link to be inserted and breaks original link

reproduce:

• create link with modal
• save
• enter edit mode
• highlight link
• open link insertion modal
• link insertion modal URL field is populated with the http://linkpath
• modify the http://linkpath
• click insert
• save
• new link is broken

in this same vein, everytime a new link is inserted over an old link a ghost of the old link remains in the html

i believe this is a bug in redactor - i fixed the bug and commited in the redactor branch, re-open if it persists
