3.* Terms and definitions: Make entry consistent with usage #23

Please add usages of the terms defined in the text or remove the entry from Clause 3.

ISO-020/-25 |   | ed | ISO/IEC Directives, Part 2, 2018, 16.5.4 Only terms which are used in the document shall be listed in the Terms and definitions clause.

Applies to 3.1.5, 3.2.10, 3.2.2, 3.2.4, 3.2.5, and 3.3.8.

I don't like it, but the required solution is clear.

The term implementation-defined behavior is never used in the spec, so I removed it from T&d.

The term unspecified behavior is only used in the definition of application-defined behavior, so I removed it and rewrote the latter definition.

All edits applied to the spec.
