Update terminology of fields in README

The overview section says "Fluentd tag names will be used for Scalyr thread names.". Thread names aren't exposed to the user in the UI... a little bit, but it's not prominent. Better to say that the tag names will be used for the log file name in Scalyr.
Update sessionInfo references to be serverAttributes - possibly rename config item.
Update config file sample to use same sessionInfo as the Scalyr API docs

config item is now server_attributes, and the README has been updated to match the preferred terminology.
