CT-502: Include gem name and version in User-Agent header #31

Note, we grab the version from the currently loaded gem, this doesn't work in tests hence the possibility of an "unknown" version.

I'll make the version change etc on Monday, I don't think there is any real risk in releasing this on a Friday but there is also no rush to get it out. Better safe than sorry I think.
