Whenever I start an MT-32 game, it seems to send the sysex information to the synth before the game starts as I see the MIDI activity light flash.

However it seems to fail as the LCD remains as it was before and the sounds that do play are not correct.

Any insight as into what's wrong?

Have you checked your midi driver setting in retroarch menu? Do you have the correct mt-32 ROM set... especially the control rom that's been patched?
