Add licenses #50

This PR adds license information to the Cookbook. As discussed previously, the scheme is multi-licensing under several common licenses. The PR has now been applied to the top of the front page of https://cookbook.scheme.org/.

Please confirm whether this is OK. @jcubic @arthurgleckler @johnwcowan

No-one has objected in 4 days, and the issue was discussed previously, so merging.
