With each import, there are a number of records that have images attached and visible in the viewer, but show the blank blue cube icon in search and browse results. Sometimes this bug appeared upon import; at other times the work had other issues (marked private, file not attached upon import) which were able to be corrected manually or via reimport, but the icon did not correct.
