Duplicate entries in "show all" view #82

http://scipy-central.org/item/show/all/ New revisions of the same submission show up as duplicates.

This is intentional, for now. But I have updated the title to be "All revisions" instead of "All submissions".

There is a way to remove duplicates, at http://scipy-central.org/item/show/all-unique-revisions/ but I haven't thought about an appropriate place for the link yet.
