Auto-link GitHub pull requests to tickets #15

Just like commits that reference a ticket should be auto-linked, PRs should be auto-linked as well.

I think the issue title of #4 was too generic. I've changed it to match the description and be specific to commits that close issues. These two features will require different code since they're different types of GitHub events.
