Fetch dataset from Spine Generic #27

"csa-atrophy" dataset is no longer hosted on "openneuro" but on "spine generic project", https://github.com/spine-generic. I have updated README and removed csa_fetch_dataset to reflect this change.

Discussion:

If user wants to run csa-atrophy pipeline on a smaller dataset he has 2 choices: either create new folder with sub-dataset or use flag -include to select subject (is it only one subject at a time?).

TODO:

Point towards a certain version of the spine generic repository?

This PR was based on branch not yet merged to master. Identical message was copied on new PR #28 .
