The Social Links Pack link 404s #2

Issue

In the readme file on wordpress.org and on github, the link for The Social Links Pack takes visitor to a 404 page.

Proposed solution

If no longer available for purchase remove link, else update link.

We're busy merging the pack into core. This should be done this week or the next ;)

PR #3 should temporarily take care of this, though it removes the links rather than creates a new one working one, temp fix.
