Not sure how it can be reproduced, but this might bring up some examples:
Go to the very last page in the reported contents page.
Click on the filters option, choose "Contains" beside content, and enter "[tex]" as the word.
Keep moving to the previous page till you find something that looks odd on the reported contents page. [You might be able to find the same examples as most of them haven't been moderated yet]
