Optimise highlighting #116

Fix and optimise highlighting dynamic insertion (see commit messages)
Correctly remove highlighting elements for particular terms
Bump patch version to 5

It is currently still possible to get stuck in an infinite highlighting loop, due to mutation updates caused by CLASSIC highlighting being incorrectly observed. I am investigating ways to definitively fix this problem.
