Cannot submit survey response

We have a survey (https://www.seasketch.org/cadiz/surveys/310) that seems to work fine until we try to submit the responses at which point we receive the following error message:

Error saving response query returned more than one row Your response is safe on this device, but will need to be submitted to the server before being counted.

Contactsupport@seasketch.orgfor assistance.

I'm not seeing this yet on my walkthrough. Any particular input needed?

No. It seemed to be doing it all the time. I'll test some more now that we're in a different venue to see if the network perhaps contributed to the issue.

From that message it is very likely something about the input. That error message sounds like it came from the database.

Weird. We tried the survey 10 times on 10 different machines and got the same error. Whatever!
