Possibly this is BUG-3416 BUG-3416 is however not a new problem. If this problem is new (or worse) since the Second Life 3.7.17 (294943) (which is the Maintenance-RC) release, I guess it is related to BUG-7433 / BUG-7436.
Glorf, you are currently using the Maintenance-RC viewer which has some known problems with not loading avatar shapes and attachment positions correctly (see BUG-7433 and BUG-7436). If you revert to the default release (download here: https://wiki.secondlife.com/wiki/Release_Notes/Second_Life_Release/3.7.17.294959) does this fix the problem?

I can easily reproduce this on Second Life 3.7.17 (294943) Sep 29 2014 17:00:36 (Second Life Release) with my Strangeling quad avatar. This is definitely not BUG-3416, this is a new problem on the MAINT-RC.
How I see myself on the MAINT-RC
How an observer sees me on the MAINT-RC

Glorf, do you know which dragon quad avatar this person was wearing? Can you give a marketplace link for it?
Here is my repro for the Strangeling avatar:
Pick up the box containing the repro avatar here http://maps.secondlife.com/secondlife/Testylvania%20Sandbox/68/96/22. Look for http://i.imgur.com/3sNvSdD.png This box contains the complete outfit.
Login Avatar A on the Maint-RC viewer Second Life 3.7.17 (294943) Sep 29 2014 17:00:36 (Second Life Release) or default release Second Life 3.7.17 (294959) Oct 1 2014 10:55:30 (Second Life Release) The viewer Avatar A is using does not matter. Avatar A will look correct to themselves on both viewers.
Avatar A, open the repro box and copy contents to inventory.
Avatar A Replace outfit with "BUG-7505 repro avatar"
Avatar A observe that you look correct on your screen, as in this image:
Login Avatar B on the Maint-RC viewer Second Life 3.7.17 (294943) Sep 29 2014 17:00:36 (Second Life Release)
Avatar B observe Avatar A. Avatar B sees Avatar A totally deformed as in the image below:
Login Avatar C on default release Second Life 3.7.17 (294959) Oct 1 2014 10:55:30 (Second Life Release)
Avatar C observe Avatar A. Avatar C sees Avatar A correctly as in the image below:
Observed Avatar A will always look correct to themselves no matter which viewer they are using. Avatar B and any observers using the MAINT-RC viewer will always see Avatar A deformed. Avatar C and any observers on default release viewer will always see Avatar A correctly.

vwr_attachment-fixes-drtvwr-388-2, Second Life 3.7.21 (296809) Nov 15 2014 12:21:27 (Second Life Test) http://automated-builds-secondlife-com.s3.amazonaws.com/hg/repo/vwr_attachment-fixes-drtvwr-388-2/rev/296809/index.html
Bug does not reproduce
Steps to Reproduce
Present at Caledon Oxbridge, otherwise behaving normally.
Actual Behavior
An avatar in dragon form has come into view, only the dragon shape is distorted and confused - apparently the prim components of the avatar are being mapped onto a normal humanoid body shape, rather than one affected by the "body crusher" animations used for quadruped avatars.
Expected Behavior
Expecting to see a normal quadruped avatar shape. (Well, not expecting so much as hoping, perhaps. This has happened with every quadruped avatar I've seen since my last viewer update.)
