Heya Meggsy,
Do you have iCloud installed on your system? There is a known problem with iCloud (see BUG-7343) which among other symptoms, causes lots of "missing from database" error messages for worn clothing items. If you do have iCloud installed, then uninstalling it will very probably fix your problem.
If you do not have iCloud installed, please try the following:
Try logging into a different region and changing your outfit and see if your avatar will rez. If your problem turns out to be limited to one specific region, then this region needs to be restarted.
If testing on a different region does not help the please can you try the following: In the top menu bar of the viewer, go to Me -> Choose an Avatar -> Classic -> Choose one of those avatars and use "Select". Your avatar should hopefully rez correctly once you have done this and you can then get dressed normally.
If this doesn't help, create a new folder in your inventory and inside this folder, place a skin, a shape, a hairbase and some eyes (non-prim). Then Right click this folder and REPLACE outfit. You must replace, not wear or add to or it wont work. If your avatar then rezzes, you can get dressed normally.

If you are still having problems with all the missing from database messages and your avatar not rezzing, please can you run a session where you change outfits a few times and reproduce the missing from database messages and then logout. Then before relaunching the viewer, zip up your logs folder and attach it to this issue using More Actions -> Attach files. Steps to Reproduce
I just logged into my viewer.
Actual Behavior
Failed to find clothing named "new pants" in database (x3). My viewer is not recognizing anything in my database. I am a cloud.
Expected Behavior
I was expecting for my avatar to load as usual.

This has been happening to me all week on and off.
