Heya Nicole,
Firstly please try logging into a different region and changing your outfit and see if your avatar will rez & your attachments will get attached correctly. If your problem turns out to be limited to one specific region, then this region needs to be restarted.
If testing on a different region does not help the please can you try the following: In the top menu bar of the viewer, go to Me -> Choose an Avatar -> Classic -> Choose one of those avatars and use "Select". Your avatar should hopefully rez correctly once you have done this and you can then get dressed normally.
If this doesn't help, create a new folder in your inventory and inside this folder, place a skin, a shape, a hairbase and some eyes (non-prim). Then Right click this folder and REPLACE outfit. You must replace, not wear or add to or it wont work. If your avatar then rezzes, you can get dressed normally and check that attachments are worn as expected.
If none of the above helps, you said you are using a different computer - is the system information you gave above from the computer where you are having the problem? If not and the computer where you are having the problem has AMD graphics, I suspect it is rigged mesh attachment that you are unable to see - see BUG-7653 for details on that bug.
