Heya Tia,
You already filed this problem at BUG-8229. If the information below does not fix the problem then you need to contact Linden Lab support directly.
Firstly please try logging into a different region and changing your outfit and see if your avatar will rez. If your problem turns out to be limited to one specific region, then this region needs to be restarted.
If testing on a different region does not help the please can you try the following: In the top menu bar of the viewer, go to Me -> Choose an Avatar -> Classic -> Choose one of those avatars and use "Select". Your avatar should hopefully rez correctly once you have done this and you can then get dressed normally.
If this doesn't help, create a new folder in your inventory and inside this folder, place a skin, a shape, a hairbase and some eyes (non-prim). Then Right click this folder and REPLACE outfit. You must replace, not wear or add to or it wont work. If your avatar then rezzes, you can get dressed normally.

Steps to Reproduce
I was standing in a sim not doing anything, then poof, I was a cloud
Actual Behavior
I'm a cloud, there seems to be no rhyme or reason for it ....... -I have tried restarting my modem - uninstalling and installing other viewers ie. firestorm, sl beta viewers -rebaking -relogging, restarting uninstalling and reinstalling the sl viewer - taking off all of my avi clothing and hair
Expected Behavior

I was expecting to continue seeing the avi I have always seen for the last 9 years!
