Heya Natash,
Firstly please try logging into a different region and changing your outfit and see if your avatar will rez. Your system info shows you are on Weapons Testing Sandbox. This region os often in a bad way because of griefers & never expect much to work properly there ;)
If testing on a different region does not help the please can you try the following: In the top menu bar of the viewer, go to Me -> Choose an Avatar -> Classic -> Choose one of those avatars and use "Select". Your avatar should hopefully rez correctly once you have done this and you can then get dressed normally.
If this doesn't help, create a new folder in your inventory and inside this folder, place a skin, a shape, a hairbase and some eyes (non-prim). Then Right click this folder and REPLACE outfit. You must replace, not wear or add to or it wont work. If your avatar then rezzes, you can get dressed normally.
