It sounds like you're talking about 2 different issues? One is your avatar not loading and the other is shutting down?
Can you please re-paste your environment into this jira? it's missing the lower part which may point to connection issues.
How are you connected to the internet? DSL or wireless?
For your avatar isssue, can you try the following:
Open your inventory and the open your "My Outfits" folder.
Choose any outfit folder there, right click it -> Replace current outfit. Your avatar should then rez.
If that does not help then go to Me -> Choose an avatar and pick any of those avatars by clicking on select.
Does your avatar then rez? If so, you should be able to get dressed in your usual avatar. If you are still having problems, please see http://community.secondlife.com/t5/English-Knowledge-Base/Troubleshooting-avatar-appearance/ta-p/1256955
If you are crashing, can you please attach a copy of your logs, the next time you crash? They can be found here: https://wiki.secondlife.com/wiki/Finding_Log_Files
