Firstly can you rule out a region problem by logging in directly to a couple of different regions & see if you still have the same problem.
If not please try having the region you were in restarted.
Is it only the single bodypart that's giving the missing from database error or does it happen when you wear other shapes,skin,hair or eyes too? If you create a new shape (open inventory and click the + button at the bottom, New body parts -> New shape) & wear this shape, does it also go missing from the database? Do you see any notifications saying "unable to save shape to central asset store" when creating a new shape?
Have you ever had Iolo System Mechanic Pro installed on your computer? This software is known to cause all shapes you create or edit to go missing from the database.
