This issue has not been a problem from me till the last viewer update by Linden Labs for the Linden viewer... I am currently using Firestorm.
Just to clarify, are you saying that you do not suffer these disconnects on Firestorm Viewer?
When logged in on the LL Viewer, in the top menu bar of the viewer, go to Help -> About Second Life. Click the "Copy to clipboard" button & paste all your system information in a comment here.

What just happened?
Disconnected while teleporting from the following places : PaleoQuest , Horizons and Linden Realms. Disconnected after finishing the 5th Quest of either PaleoQuest or Horizons going to the Prize Room , Riding the Monorail or teleporting to the main hallway in Paleo equals and automatic disconnect. Teleporting from Linden Realms anywhere to Tyrahs workshop equals and automatic disconnect. Attempts to connect me to the area but then kicks me off.
What were you doing when it happened?
Horizons 5th Quest finishing the final phase to be teleported to the prize room leads to automatic disconnect. PaleoQuest riding the monorail to exit to the main hallway leads to being kicked out of the monorail and being electrocuted or teleporting to the main hallway after the 5th Quest leads being disconnected. LindenRealms - Teleporting from the Desert , Swamp , Volcano to Tyrahs workshop equals a automatic disconnect , anywhere near or next to Tyrahs workshop does not disconnect me. However Tyrahs workshop is no longer visible nor is the terrain in or around Tyrahs workshop.
What were you expecting to happen instead?
Linden Realms expecting to end up at Tyrahs workshop Horizons expecting to end up in the Prize room. PaleoQuest expecting to be dropped off or teleported to the monorail exit at the main hallway.
Other information
Disconnects from Teleporting maybe at random but not random when exiting PaleoQuest / Horizons. Paleoquest on completion or incompletion exiting The Talpa Caves to the main hall = Disconnect.. Horizons finishing 5th Quest and being teleported to Prize room = Disconnect. Linden Realms - Land Mass at beginning and or at Volcano goes missing the entire land mass. Teleporting to Tyrahs workshop = Disconnect. Tried working my way around it by teleporting to the Portal Park - entrance to either Linden Realms / Horizons or Paleo = Automatic Disconnect.
I have had no more than maybe 1 to zero disconnects on average, "before" Linden Labs did an update to their viewer.
I have zero packet loss. I have rebooted the modem and the computer. I have also cleaned my Cache.
I have given Firestorm access to inbound and outbound rule settings via the firewall. Also a fresh reinstall of Firestorm with a reboot of the computer.
I have changed my IP settings to Google as recommended. I have also have run and tested the Ping and it is sitting at 3 ms.
My Ping sits at 100 or less while running Second Life. I, am on a Fiber line and have a direct connection to the modem to the computer. It is not a Anti-Virus issue on my end, already checked and verified. I have updated my video card drivers.
None of the changes have affected the disconnect issue.
I have tried going around the teleporting disconnect issue by exiting after finishing the 5th quest before teleporting or being teleported by using the home button and then tried re-entering the portal minutes later only to be disconnected.
