adjust logo position and objects to match

The majority of the changes to index.php are from lines 141 to lines 245 (div class="workspace-split mod-horizontal mod-left-split") the changes in app.css that matter are specific to the logo, the vault title, and class containing the search and file tabs.

the rest of the changes is just formatting changes. I let vscode run a formatter on both app.css and index.php
