docs: Rewrite README #205

Lately I've felt that Vocab's README has been lacking information for consumers to successfully use the framework, as well as general guidance on recommended practices.

This PR addresses these issues in a few ways:

Additional uplifts such as fixing grammatical mistakes, updating code blocks and some re-organization have also been performed.
