[x] remove parentPattern as it wasn't being used
[x] extract _matchSubArray and _matchEllipsis from _matchNode
[x] combine constraints, placeholders and ~indexes~ into a single context variable
[x] add comments
[x] extract non-match functions such as applyRule, defineRule, etc. from matcher.js into a new file
