update tests to be compatible update README examples

Thanks @JoshRagem. Looks like the Travis build/tests are not passing now. Not sure if you saw that?

@otisg I did see that, but aside from node 4 saying there is a syntax error, it seems like travis is no longer injecting encrypted environment variables for external PRs https://docs.travis-ci.com/user/pull-requests/#pull-requests-and-security-restrictions

the "syntax error" on node 4 and 5 I have not looked into, but winston 3 doesn't support them anyway: "engines": { "node": ">= 6.4.0" },
