export the start date field #281

We are not currently exporting the start date. I'm guessing I left this off when I re-ordered and culled the output. But this means that data is lost when people save. So this is a pretty urgent bug fix need.

I'll fix this and deal with #250, hopefully today.

My bad. This is there. I just didn't see it in the CSV output. Closing.
