[x] Model placements
[x] Link the preferred placement of a name to parent and assigned_by
[x] Make the first placement of a name always preferred
[x] Link alternative (non-preferred) placements; create preferred placement first if necessary
[x] Add "prefer placement" button only for curators

Decided to detach this task because it has much lower priority: #123
