Tell us about your request! We should set up a customer satisfaction survey for patients on Serenity so that we can measure their happiness ratings as well as collect feedback after each patient visit

Which app does this concern? Provider portal

Describe the solution you'd like When a patient's visit ends, they should get an SMS and email notifying them to fill the a CSAT survey. There, they can fill the form to provide their feedback. We should do this with Delighted

Describe alternatives you've considered

  1. Qualtrics (Delighted is from Qualtrics though)
  2. Typeform
